(a) In O.P.D : Each & every department has its own Out-door clinic.
There is also a minor operation theatre in out-door department Each & every department has been provided with the clinical demonstration class room in the out door.
(b) In indoor : Each & every department has been provided with clinical demonstration rooms adjoining the wards.

REGISTRATION, MEDICAL RECORDS & STATISTICS DEPARTMENT

Central and/or Departments

For in patients : The patient is advised admission in the OPD by the competent authority . The admission slip is brought to the CR counter for getting the CR No. The Registration Clerk will charge the admission fee from the patient and will make the following entries in the admission registrar-OPD No., name of the Patient, fathers name/ Husband name with full postal address , income, age, & sex. After making these entries the registration clerk allots CR. No. on the admission ticket which is used for further investigation to be carried out in the hospital . As a rule, no patient can be admitted in the ward without obtaining CR No.

Daily census of indoor patient is done by the concerned clerk . He/ she records the list of discharged patients from each ward as well as list of cases of death . He/She also receives the files from the concerned residents and then hands over the files to the Asstt. Librarian of CR office.

He/ She will also prepare a monthly report for all the Deppt. Showing the list of case sheets of discharged and expired patient not received in the CR office . He will also issue reminder to concerned Deptts. This ensures that entire records of the discharged and expired patient is received in

the CR office well in time . On the basis of census of previous day he will prepare daily in- patient’s reports which is displayed on the board along with the OPD report.
Birth & death clerk collects daily information- relating to deaths from emergency and births from Obst. & Gynae deptt. All births & deaths are entered in the CR office registers and true copies of entries are sent to local Registrar Birth & Death , M.C. Patiala

For O.P.D. patients: : There are 21 out patients deptts. functioning in is hospital in addition to emergency deptt .

All the patient go to concerned OPD. Each patient his/her with OPD No. from the concerned pharmacist . Then the doctors examine the patient and advise necessary treatment or admission as the case may be . CR office clerk daily visits all OPDs for conducting OPD patient census His daily report of previous day is displayed on the boards.
